Functions as a key member of the Liver Transplant Team, coordinating with nurses, physicians, and allied health professionals to optimise patient care.

Coordinates scheduling of inpatient procedures and imaging for both routine and urgent Liver Transplant cases.

Conducts daily rounds on Liver Transplant patients with attending surgical team.

Performs comprehensive daily chart reviews, assessing systems, overnight events, lab results, medications, and analgesia.

Ensures appropriateness of lab orders and coordinates urgent blood draws as necessary.

Facilitates necessary consultations for Liver Transplant patients with various specialities such as Endocrinology and Nephrology.

Assists with bedside procedures including abdominal drain removal and wound care.

Assessed patients’ needs for discharge planning and utilised resources promptly.

Served as liaison between patient, physician, and payer throughout care continuum.

Determined appropriate level of care for patients upon discharge.

Ensured completion of all documentation related to discharge planning.
