• Assign correct ICD-10-CM codes based on medical documentation to reflect diagnosis and procedures.
• Carefully review physician notes, patient medical records, operative notes, discharge summaries, lab results to determine correct codes.
• Analyze medical records to ensure coding accuracy and completeness.
• Ensure Clinical documentation supports assigned codes
• Follow official coding guidelines, payer policies, and regulatory requirements like HIPAA.
• Enter codes into electronic health records (EHR) or coding software systems (e.g., HCC tools).
• Stay up-to-date with changes in coding software tools and code sets.
• Perform internal audits of coded data to identify and correct errors or inconsistencies
• Correct coding errors and assist in implementing quality improvement measures.
• Communicate with supervisor to clarify missing, conflicting or ambiguous information in documentation
• Stay current with ICD-10 updates, coding changes, and evolving healthcare regulations
