STEMI Audit instances of Delay in Urgent PCI Arrangement for STEMI Cases was identified, Analysis revealed causes involved ECG Delay by Nursing Staff, Delay in Physician Assessment. As a remedial Action, a Policy was established for Nursing Staff to Ensure ECG of patients complaining of chest pain Within Ten Minutes of Arrival and Physician to Assess the ECG within 15 Minutes. This Significantly reduced the Door to Balloon Time. Re-Audit verified the Positive Impact.
Stroke Audit, instances of missing the thrombolysis Window were identified because of delays originating from multi-disciplinary Interaction and response. Therefore A Urgent Response Code was Established with clearly communicated guidelines to a Specifically drafted Multi-disciplinary Stroke Team. Upon Arrival of a Suspected Stroke Patient, After Initial Assessment by Emergency Physician, Stroke Code is Announced, The Patient is directly taken to the CT and the Stroke Team takes Over. Since Implementation of Stroke Code, reaudits revealed not even a single instance of Missing the Thrombolysis window.
